This is a voluntary, employee-paid, supplemental vision care plan. Note that this policy is completely separate from the one (1) eye exam/year offered through the medical insurance plan.
FMLA Process Flow Chart
Beginning 12/21/2017, supervisors and payroll reps are no longer initiating FMLA cases. HR will be taking over leave management.
