Outstanding Family Caregiver Award
Criteria:
- Age: The family caregiver is caring for someone who is 60 years of age or older in the community.
- Advocacy: The caregiver speaks out on behalf of the special needs and interests of family caregivers, thus raising awareness in our community.
- Positive Attitude: They have a positive attitude and are genuinely warm and generous of spirit.
- Willing to receive help and support: They recognize their limits and are able to graciously receive help from others.
- Positive impact of care: The care given provides the highest quality of life possible for the person receiving care.
All nomination forms must be turned in or postmarked by April 1, 2021.
