Secure and verifiable identification is required on all new titles, duplicate titles, new registrations, new temporary registration permits, and new placards. The identification must be provided by the owner of record or a power of attorney presented by the agent. The agent must have secure and verifiable identification to complete the transaction. Commercially owned or lien holder transactions require a letter of authorization.

Criteria

Disability plates and/or placards are issued to a vehicle or individual when someone is disabled with qualifying criteria as listed below:

  • Persons who cannot walk two hundred feet without stopping to rest
  • Persons who cannot walk without the use of, or assistance from, a brace, cane, crutch, another person, prosthetic device, wheelchair, or other assistive device
  • Persons who are restricted by lung disease to such an extent that the person's forced (respiratory) expiratory volume for one second when measured by spirometry is less than one liter, or the arterial oxygen tension is less than sixty mm/hg on room air or at rest.
  • Persons who use portable oxygen
  • Persons who have a cardiac condition to the extent that the person's functional limitations are classified in severity as Class III or Class IV according to standards set by the American Heart Association
  • Persons who are severely limited in their ability to walk due to an arthritic, neurological, or orthopedic condition

Disability plates or placards enable the driver to park in the specially marked parking zones. Placards must be removed from the rear view mirror or window when the vehicle is motion.

Registration Types

Three types of registration are available:

  1. Permanent license plates only - Application fees are calculated the same fees as for license plates and are issued to a person with a disability who owns and drives a vehicle.
  2. Permanent placards - For persons with permanent disabilities who do not own or who own more than one vehicle. The placard will be issued which is to be placed inside the vehicle when the person with a disability is riding. The placard is transferrable from one vehicle to another. This does require recertification every three years.
  3. Temporary placards - These are issued for a time period of up to three months to a person with a temporary disability.

Once approved you may be issued the following combinations: (1) set of plates, (2) a set of plates and a placard or (3) 2 placards.

Application

A DR2219 Persons with Disabilities Parking Privileges Application must be completely filled out in front of a medical professional. The completed application is required to obtain the placards and/or plates. Completed applications may be presented in person at one of our locations or mailed to:
