The Healthy Communities Program helps families with children under age 21 and pregnant women apply for Medicaid and/or the Child Health Plan Plus (CHP+). The program also helps families on Health First Colorado (Medicaid) and/or CHP+ understand their benefits and find medical, dental, vision, mental health, and specialty services.

Office hours: 

Because our staff work part-time, there may be regular office hours when our office is closed because of limited staffing or for training or meetings. We encourage you to call ahead if you plan to visit to make sure that someone will be available to help you.  We are located off of the main waiting room in the Health Department in Fort Collins.

Mondays - 8:30 am -4:00 pm

Tuesdays - 1:00 pm -4:00 pm. Please call for morning appointment availability.

Wednesdays - 8:30 am -12:00 noon. Please call for afternoon appointment availability.

Thursdays - 8:30 am -4:00 pm

Fridays - 8:30 am -4:00 pm
