ALS (Amyotrophic Lateral Sclerosis)

Provides support groups, equipment loan, library loan, information, resource list and respite care grants.

Call the local office for information on the support groups closest to you.

Alzheimer's Disease

Provides information, education, respite grants, Safe Return program, Savvy Caregiver program, learning institute for health professionals and various support groups in Estes Park, Fort Collins and Loveland.

Arthritis

Provides local class schedules, an advocacy program, physician referral list and research information.

Brain Injury

Provides support groups, resource directory, advocacy, community education/prevention programs, conferences, volunteer programs and information on adult camps.

Cancer

Local “I Can Cope” education program at Poudre Valley Hospital, 970/495-8383.

Colorado Women’s Cancer Control Initiative, 970/498-6751.

Patient Navigator Program, a private, non-profit program provides free information and referral. Goal is to improve access to care. 970/402-0779.

Diabetes

Provides information on prevention, research, community programs and local events, nutrition, weight loss, exercise, advocacy, list of recognized education programs and a list of recognized physicians.

Heart Disease

Provides information on warning signs, healthy lifestyles, heart and stroke encyclopedia, advocacy, publications, resource lists and CPR class locator.

Heartwatchers – organized fitness class for cardiac patients at McKee Medical Center, 970/635-4025.

Mended Hearts – monthly support group that meets at Poudre Valley Hospital. Call 970/593-9773 for more information.

Kidney Disease

Provides information and referral, financial assistance grants and education.

Lung Disease

Provides education on asthma, COPD, etc., fact sheets, workshops, listing of pulmonary rehabilitation groups and information on local support groups.

Provides information and referral, case management and counseling for all ages.
Also have Loveland and Estes Park locations.

Multiple Sclerosis

Provides counseling, peer support, support groups, information and referral, limited financial assistance, exercise classes, case management with ILEAP (Independent Living and Advocacy program), and self-help groups in Loveland and Fort Collins.

Provides information on risk factors, recovery, prevention, caregiver and families, women and stroke, African Americans and stroke, children and stroke. Stroke Smart magazine available.

Provides a help line, peer visitation, survivor support groups, community education, caregiver services, aphasia group, speakers bureau, summer camp, rehabilitation classes and yoga video.

Information on recovery, warning signs, life after stroke, heart and stroke encyclopedia, health care professional section, family caregiver information, Stroke Connection magazine and support group locations.
