Motor Vehicle Checklist

We accept CASH OR CHECK ONLY - an ATM is located in the Fort Collins lobby for your convenience.

We have a new lobby management system in Fort Collins and Loveland! When entering these offices please take a number. The new system will identify and prioritize our customers.

CURRENT INSURANCE is required on all temporary or permanent registrations. (Not trailers) Please have a copy of your insurance card ready. If you need to contact your agent, our customer service phone and fax are available. Please see fax numbers above for the correct location. For assistance see our information desk.

Effective July 1, 2006, Secure and Verifiable Identification is required on all New Titles, Duplicate Titles, New Registrations, New Temporary Registration Permits and New Placards. The identification must be provided by the owner of record or a power of attorney presented by the agent. The agent must have Secure and Verifiable Identification to complete the transaction. Commercially owned or lien holder transactions require a letter of authorization.

Renewals

Please take a number. Proof of Insurance & Emissions may be required. $10 late fee on expired registrations is applicable. Owner of Record must have identification available or an agent must have a power of attorney from the owner and their id present.

Temporary Permits (you will need proof of insurance)

In State Purchase

____ Title (with completed odometer section on vehicles under 10 years old)
____ Dealer Invoice or Bill of Sale Notarized or with Perjury Clause (dollar amount and date are not listed on the title)
____ Proof of Sales Tax paid if applicable.
____ Emissions Test (see below)
____ Proof of insurance
____ Owner must present identification or agent must have power of attorney with identification.

Out Of State Transfer

____ Title   OR
  1. Current registration in owners name may be surrendered. (No Colorado title issued)
  2. Registration over 6 months expired-copy of the title is acceptable (No Colorado title issued)
____ DR2395 VIN VERIFICATION - Can be obtained by taking your vehicle to any major automotive facility. Form is available in the forms holder.
____ Dealer Invoice or Bill of Sale Notarized or with Perjury Clause (if the dollar amount and date are not listed on the title)
____ Proof of Sales Tax paid if applicable.
____ Emissions Test (see below)
____ Proof of insurance
____ Owner must present identification or agent must have power of attorney with identification.

Other Important Information

____ EMISSIONS TESTS are required on diesel vehicles. Effective January 1, 2007, gasoline vehicles are no longer required to have a basic emissions test to be used in Larimer, Weld, or El Paso counties. If the vehicle is used over 90 days a year in the Enhanced areas, (Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, and Jefferson county) an Enhanced emissions test is required. ____ DO YOU HAVE PLATES TO TRANSFER? Notify the Technician with the previous plate number or registration.
____ FINANCED OR LEASED? When your paperwork is processed in our office a Title Complete Notice will be mailed to you. If you have not received anything, please see our Information Desk to verify the status of the documents. You may need to contact your dealer or financial institution.
____ WEIGHT SLIP for MODIFIED vehicles, HEAVY TRUCKS and homemade TRAILERS weighing over 2000 lbs. or if weight is not listed on your ownership documents.
