1. Letter must be on company letterhead
2. Date
3. Dealer Number or Lienholder ID number
4. State what the agent is authorized to do on behalf of the company
5. Name of the Authorized Agent(s)
6. Signature of company representative
7. Original, faxed or photocopy is acceptable.
