Documents to bring when applying for AND, OAP, QMB, SLIMB or QI-1
Bring as many documents as possible. If you do not have all of your paperwork in order you can gather it after the first interview.
- PROOF OF AGE, such as
- birth certificate or
- baptismal certificate or
- document 10 years old with birthdate
- PROOF OF U.S. CITIZENSHIP, such as
- birth certificate, or
- citizenship records, or
- social security card
- PROOF OF COLORADO RESIDENCY, such as
- any photo I.D. with your address, or
- rent receipt
- PROOF OF YOUR GROSS INCOME AND YOUR SPOUSES'S/PARTNER'S GROSS INCOME, such as
- payroll stubs
- verification of social security and SSI
- proof of other income or checks you receive (child support, alimony, self-employment, financial aid)
- PROOF OF RESOURCES (bring all of these)
- bank statements
- value of motor vehicles; if more than the vehicle
- tax statement for real estate property
- life insurance letter showing face and cash value
- burial funds or policies
- IRA/stocks/bonds
