  2. How do I file a complaint or make an inquiry?

    The preferred method to file a complaint, to be sure you provide all of the information we will need, is to download the Dept of Human Services Complaint form. You can submit this form electronically or print it and mail it to:

    You can also contact the Executive Assistant to the Director at (970) 498-6311, or send e-mail. You need to be prepared to provide the following information:

    • Your name, address, phone number and email.
    • A brief description of your complaint
    • Your involvement with the complaint
    • What specific outcome you desire
    • The name(s) of the Larimer County Department of Human Services staff person(s) with whom you have discussed your concerns, as well as the outcome of those discussions.
