The process to create the five-year CHIP began after Colorado passed the Public Health Reauthorization Act in 2008. This legislation requires local public health agencies to create a health improvement plan, based on a community health assessment and with community input about priorities, at least every five years.
The 2014 - 2018 Community Health Improvement Plan (CHIP) reflects the vision and values of the community. Throughout each step of the assessment and planning process, community partners emphasized the importance of focusing on prevention and employing evidence based strategies for promoting and improving health equity. For our community, achieving health equity requires understanding where disparities in health exist and a commitment to working to reduce them. It will take the entire community to create a healthy community for Larimer County residents to live, work, learn, and play.
