Services Provided

  • Certificates for all births and deaths occurring in Larimer County are issued.
  • In addition to Larimer County births, certified copies of births occurring anywhere in Colorado from 1900 to present are issued.
  • Certified copies of death certificates are available only for deaths which occurred in Larimer County. Burial permits (Authority for Final Disposition) are issued to funeral homes.

Birth Certificates

  1. How Do I Get A Birth Certificate?
    Certified copies of Birth Certificates are available for all births occurring in Larimer County, and for births occurring anywhere in Colorado from 1900 to the present. For in-state births occurring outside Larimer County before 1900, contact the County where the birth occurred or the State Health Department (303) 692-2200. Certified copies of birth certificates are issued on Banknote paper. They are not issued with a raised seal, however a state seal is embossed in the paper. A raised seal is not needed for travel. A certified copy is considered a legal copy of your birth certificate. A hospital issued birth certificate cannot be used for legal purposes. You will need to supply us with a photo ID. If one is not available please contact our office at 970-498-6710 for a list of approved identification sources. Proof of relationship will need to be submitted if the birth certificate is for someone other than yourself or parent.

  3. How Much Does it Cost?
    $17.00 for the first certified copy, $10.00 for each additional copy of the same record ordered at the same time. Checks should be made out to Larimer County Department of Health & Environment. If you choose to order by credit card you will be charged an additional fee of $6.00 (Credit card orders will only be taken by faxed orders. No walkins or phone orders.). When paying by credit card, you can also choose to have your certificates mailed via Federal Express for an additional fee of $17.50 for regular delivery or $27.50 for Saturday delivery.
  4. Who can get a certified birth certificate?
    Someone with a direct and tangible interest: One's self or immediate relative: mother or father (if father is listed on birth certificate). Maternal Grandparents, Paternal Grandparents (if father is listed in the certificate), siblings or someone who has legal guardianship with paperwork showing such. Proof of relationship will need to be submitted if the birth certificate is for someone other than yourself or parent. Please call 970-498-6710 for any other requests.
  5. What is proof of relationship?
    If you are obtaining a birth certificate for a sibling, parent, grandparent, etc. you will need to show chain of relationship. Examples are: A sibling could provide their birth certificate to show same parents. If you are trying to get your grandparents birth certificate you will need to show your birth certificate, plus your parents birth certificate to show chain of relationship.   6. How can I obtain a certified birth certificate?
    You can either come to our office in person, send in a written request, or call in an order by credit card. If you come to our office in person there is a short form that needs to be filled out and you would need to present a photo identification card. If faxed in using a credit card a phot ID will need to be faxed or mailed in to us.

    If you mail in a request, the following information needs to be included for birth certificates requests:
    person's name appearing on certificate
    date and place of birth,
    parents' names along with mother's full maiden name,
    purpose for the copy,
    purpose for the copy (and proof of this documentation is needed, see above),
    your relationship to the person,
    proof of relationship,
    your signature,
    check made out for the number of copies you are requesting,
    a copy of your driver's license
    Please allow 10 days for mail delivery.

Death Certificates

  1. How Do I Get a Death Certificate?
    Death Certificates are available only for deaths occurring in Larimer County. For other Colorado deaths, contact the County where the death occurred or the State Health Department (303) 692-2200. Certified copies of death certificates are issued on Banknote paper. They are not issued with a raised seal, however a state seal is embossed in the paper. Proof of relationship is required.

    Someone with a direct and tangible interest. Relatives are wife, mother, father, child (if they are listed on the death certificate). If they are not listed on the death certificate they must show proof of relationship. Maternal grandparents, paternal grandparents, siblings if they can prove chain of relationship. Death certificates can also be issued for the purpose of banking (example: investments , trust funds), real estate, motor vehicle transfer and other legal purposes, but proof needs to be submitted with request. Law offices will need to submit a written request on their letterhead along with proof of relationship.

    Changes in Birth and Death Certificates must be made through the Colorado Department of Public Health and Environment, Certification Section, Vital Records Division:

  1. How Do I Get to the Vital Statistics Office at the Department of Health and Environment?
    The Vital Statistics Office is on the first floor of the Fort Collins Office of the Department of Health and Environment, on your left inside the second set of glass doors. The address is:
    1525 Blue Spruce Drive, Fort Collins, CO, 80524-2004.

    To get to the Health Department:

    From I-25: Take I 25 to the Mulberry Street (Highway 14) exit heading West. When you get to College Avenue, turn Right (North) and follow directions below:

    From College Avenue (Highway 287), downtown Fort Collins: From downtown, go North on College until you get to the light at College and Conifer (JAX surplus is on the SE corner). Turn Right on Conifer and take a Left at Blue Spruce Drive. Go North about two Blocks until you see the sign on your left indicating the Larimer County Offices. The Dept. of Health & Environment is the second building, 1525 Blue Spruce Drive.
